Transit Visa for Crew Members
SUBMIT ORIGINALS AND ONE SET OF COPIES OF EACH DOCUMENT:

1. Two Schengen Visa Application forms completed and signed.

2. Two recent pictures, passport size pictures (white background) glued to the application forms.

3. Passport  (must be valid for a minimum of three months beyond the expected day of return).

4. Permit to return to the United States of America in the form of: a resident card or visa.

5. Letter from the cruise line by showing the job title of each worker, port of first entry, proof of return, period of transfer, itinerary, and name of the ship.

6. Letter from the company assuming all the responsability and medical insurance.

7. Invitation letter from the Port Agent in Spain with the following information:

    Name of the Seaman
    Place and date of Birth
    Passport number
    Validity of passport (issued and expiration date)
    Seaman’s book number
    Validity of Seaman’s book
    Job title of the Seaman
    Date of entry into Spain and Airport
    Vessel name and its flag
    Port of embarkation
    Duration of stay on the vessel
    Port of disembarkation
    Date of return to home country
    Itinerary of arrival and return to home country
    Name and address of agent in home country
    Confirmation to take responsability of transferring the Seaman to and from Spain

8. Non-refundable fee, payable with Money order (to the order of: CONSULATE GENERAL OF SPAIN)
 
Honorary Viceconsulates of Spain are not allowed to issue visas.
