The type of appointment you need (visa or NIE) Name and surname of the applicant, date of birth, Passport Nº, address, email address and telephone number If applying for a visa, the type of visa you need (e.g. Study visa) If applying for a NIE, the reason for which you need it (e.g. inheritance) Availability:
NOTE: If you state that you don't have a preferred
day and time slot, you will be automatically given the first available
appointment.
